Registration Forms

To register for Faith Formation classes, proceed as follows:

• Download one Family Registration Form for each family.
• Download one student form according to grade level needed.
• Fill in all information and return to the parish office with payment.
• If you have questions or trouble downloading the forms contact the person in charge of the appropriate grade level program.

The registration forms listed below must be filled out annually.

2009 Family Registration Form

Pre-K - 5^th grade registration form
